Informed consent and acknowledgment of services

Please read this consent form carefully before receiving any DarDoc service. By confirming acceptance, you provide informed consent to the delivery of your booked service.

1. Scope of Services

DarDoc Health Technologies Limited, including its affiliates and subsidiaries ("DarDoc," "the Company," "we," or "our"), facilitates the provision of a range of healthcare and wellness services, including but not limited to intravenous (IV) drip therapy, blood testing, injections, peptide administration, pharmaceutical delivery, virtual medical consultations, and such other healthcare and wellness services as may be offered by the Company from time to time (collectively, "Services").

Services may be rendered at a location designated by the User ("Home Services") or through a remote telecommunications platform ("Virtual Services"), as applicable to the nature of the service booked.

Services may be performed by licensed healthcare professionals directly engaged by DarDoc or by authorized third-party healthcare providers acting on behalf of and under arrangement with DarDoc ("Healthcare Provider(s)"). The engagement of a third-party provider shall not diminish the applicability of this consent, and all terms herein shall apply irrespective of whether Services are delivered by DarDoc or its authorized Healthcare Providers.

The specific nature and scope of the Service booked by the User ("the Booked Service") has been described during the booking process. The User is encouraged to direct any inquiries regarding the nature, scope, or suitability of the Booked Service to DarDoc prior to confirming the booking.

2. Informed Consent

By accepting this form, the User hereby provides informed consent and acknowledges the following:

(a)The User has been informed of the general nature, purpose, and scope of the Booked Service as described during the booking process or by the attending Healthcare Provider.
(b)The User has been afforded a reasonable opportunity to ask questions regarding the Booked Service and has received satisfactory responses to such inquiries, if any.
(c)The User understands the information contained in this consent form and has not been coerced, unduly influenced, or otherwise compelled to provide this consent.
(d)The User voluntarily authorizes and consents to the delivery of the Booked Service by a licensed Healthcare Provider furnished by or on behalf of DarDoc Health Technologies Limited, its affiliates, or subsidiaries.

3. Disclosure of Medical and Health Information

The User represents and warrants that they have provided accurate, complete, and current information regarding:

All known existing medical conditions, diagnoses, or ongoing treatments;
Any known allergies to medications, substances, latex, or other materials;
Any history of adverse reactions to medications, treatments, or medical procedures;
All medications, supplements, or homeopathic remedies currently being taken;
Pregnancy, breastfeeding status, or the possibility thereof, where applicable.

The User acknowledges that the failure to disclose, or the provision of inaccurate or incomplete health information, may materially increase the risk of adverse outcomes, complications, or injury. DarDoc and its Healthcare Providers shall not be held liable for any adverse event arising directly or indirectly from the User's failure to provide accurate and complete information.

The User further undertakes to inform the attending Healthcare Provider of any changes to their health status prior to the commencement of any Service.

4. Risks and Complications

The User acknowledges that all healthcare and wellness services, regardless of nature, carry inherent risks. Depending on the specific Service rendered, risks may include, without limitation:

Discomfort, bruising, swelling, or pain at the site of any injection, cannulation, or blood draw;
Minor or prolonged bleeding;
Dizziness, lightheadedness, nausea, or vasovagal response;
Localized or systemic allergic reactions, including anaphylaxis;
Infection or inflammation at the site of a procedure;
Adverse or idiosyncratic reactions to medications, supplements, or substances administered;
Vein inflammation (phlebitis), infiltration, or extravasation;
In rare cases, cardiac complications, metabolic disturbances, or other serious adverse events.

This list is not exhaustive. The severity, likelihood, and nature of potential risks vary according to the specific Service, the User's health profile, and other individual factors. The attending Healthcare Provider may communicate additional risks specific to the Booked Service at the time of the visit.

If any adverse reaction occurs during the delivery of a Service, the Healthcare Provider will take clinically appropriate steps and may recommend further medical care. The User acknowledges that no guarantee, warranty, or assurance has been made regarding the outcome or results of any Service.

5. Potential Benefits

Potential benefits of DarDoc's Services may include, without limitation:

Access to licensed professional healthcare and wellness services at the User’s preferred location, reducing the necessity for hospital or clinic attendance;
Timely and convenient delivery of treatments, diagnostics, and pharmaceutical products;
Personalized care administered by qualified Healthcare Providers;
Enhanced continuity of care through coordinated home-based or virtual services.

The User acknowledges that outcomes and benefits may vary between individuals and are neither guaranteed nor assured.

6. Home Service and Virtual Service Acknowledgment

6.1 Home Services

Where Services are delivered at the User's designated location, the User acknowledges and agrees to ensure that:

The designated location provides a safe, clean, and hygienic environment suitable for the delivery of healthcare and wellness services;
Adequate space, lighting, and access to handwashing facilities are available to the Healthcare Provider;
All animals are secured and the environment is rendered safe for the Healthcare Provider for the duration of the visit.

6.2 Virtual Services

Where Services are delivered via virtual consultation, the User acknowledges and agrees to ensure that:

A stable and adequate internet connection is available for the duration of the consultation;
The User is situated in a private environment suitable for a confidential medical consultation.

6.3 Right to Discontinue

DarDoc and its authorized Healthcare Providers reserve the right, at their sole discretion, to decline to commence or to discontinue any Service if the environment or circumstances are deemed unsafe, unsuitable, or medically inappropriate. Such a determination shall be made by the attending Healthcare Provider in their professional judgment.

7. Additional Consents

This consent form constitutes a general consent to the Booked Service. The User acknowledges that the attending Healthcare Provider may, at the time of the visit, request additional or supplementary consent for specific procedures, tests, treatments, or modifications to the Booked Service that are deemed clinically necessary or appropriate.

The User shall have the right to accept or decline any such additional consent. No Service beyond the scope of the Booked Service or this consent form shall be performed without the User's express additional consent, except in circumstances of medical emergency where obtaining consent is not reasonably practicable.

8. Right to Refuse or Withdraw Consent

The User understands and acknowledges that they retain the right to refuse, decline, or withdraw consent to any Service at any time prior to or during the delivery of such Service, without prejudice. The exercise of this right shall not affect the User's entitlement to future Services from DarDoc.

The Healthcare Provider likewise reserves the right to decline or discontinue a Service where, in their professional clinical judgment, proceeding would be medically unsafe, contraindicated, or otherwise inappropriate.

9. Data Privacy and Health Information Sharing

9.1 DarDoc Health Technologies Limited, including its affiliates and subsidiaries, is committed to the protection, confidentiality, and security of User health information in accordance with all applicable UAE federal and emirate-level healthcare regulations, data protection laws, and any other legislation governing the processing of personal and health data.

9.2 In compliance with applicable regulatory requirements, the Company and its authorized Healthcare Providers may participate in government-approved health information exchange systems operating within the UAE. Where required or permitted by law, User health information, including but not limited to consultation records, treatment data, diagnostic results, and medical records, may be securely transmitted through such systems to authorized healthcare providers, regulatory authorities, and government entities for the purpose of supporting safe, coordinated, and continuous medical care.

9.3 By accepting this consent form, the User acknowledges and consents to the collection, processing, storage, and secure sharing of their health information by DarDoc and its authorized Healthcare Providers through such systems and in such manner as is required or permitted under applicable UAE law.

9.4 Access Restrictions

Access to User health information shall be strictly limited to:

(i)The attending Healthcare Provider;
(ii)Authorized personnel of DarDoc, its affiliates, subsidiaries, and authorized third-party service providers with a legitimate need to access such information; and
(iii)Regulatory bodies and government authorities as required by law.

10. User Acknowledgment and Consent

By confirming acceptance of this policy, the User hereby acknowledges and agrees that:

(a) The User has read, understood, and had the opportunity to ask questions regarding the contents of this consent policy;
(b) The User has provided accurate and complete health information as required under Section 3 herein;
(c) The User has been informed of the potential risks and benefits associated with the Booked Service;
(d) The User voluntarily and without coercion consents to receive the Booked Service, to be delivered at the User’s designated location or via virtual means, as applicable, by a licensed Healthcare Provider furnished by or on behalf of DarDoc Health Technologies Limited, its affiliates, or subsidiaries;
(e) This consent shall remain in effect for the Booked Service and any reasonably related follow-up care unless expressly revoked by the User in writing.
